This is where the fun begins - Paperwork!! Parents interested in enrolling at Kid Angles are asked to fill out the following forms to ensure our compliance with licensing requirements. We also ask for additional information so that we can care for you child safely and have contacts in case of emergency.

These forms are available to print out and take to your pediatrician's office. Both forms are required for enrollment at Kid Angles. Please print out both pages!
